STEPS FOR SPORTS CLEARANCE/REGISTRATION

1.  The child must have a physical on file within the past 12 months.
    
2. Register your child on  (formerly FamilyID). ArbiterSports will open on or about 30 days prior to the start of the season.
    
3. Once registered you will receive a confirmation email from ArbiterSports.
    
4. Once the nurse has reviewed the registration you will receive an email indicating APPROVED (cleared to begin) or NOT APPROVED (nurse will indicate which missing documents your child needs for clearance)

DETAILED STEPS FOR SPORTS CLEARANCE/REGISTRATION

1. All student-athletes must have a physical on file, with the building nurse, that was conducted with in the past 12 months. The physical is valid through the last day of the month in which the physical was conducted.
    
2. The ³Ô¹ÏºÚÁÏÍø School District offers free physicals at the end of the school year (late May or June). Dates and times will be communicated through our various school district communication venues (Parent Square, Twitter, Instagram, Facebook)
    
3. If you receive a physical from your personal physician, the student/parent must bring a copy of the physical exam form to the school nurse. If ArbiterSports is open, you can scan and attach the physical in Arbiter Sports for the school nurse to review. 
    
4. Registration for sports will open on or about 30 days prior to the start of the respective season. To register the child the parent will log onto Family ID and fill out online registration                      forms  (formerly FamilyID).

5. If the student has any health issues (i.e.: asthma, any other health issue or past injury), forms can be picked up in the Health Office or found below under Related Files. These forms must be filled out prior to the nurses clearing them for tryouts.

   If your child is missing any paperwork for their clearance, you will receive an email from the school nurse through ArbiterSports indicating what is still needed.
    

6. Once cleared through the school nurse, parents/coaches will receive an email from ArbiterSports to verify clearance.

PLEASE NOTE:  If the 12-month period for the physical expires during a sports season, participants are able to complete the season.
